Cardiac Manifestation of Mucocutaneous Lymph Node Syndrome (Kawasaki Disease)

Xiushui Ren, MD and Rajesh Banker, MD

Division of Cardiology, California Pacific Medical Center, San Francisco, California

Manuscript received January 12, 2009; accepted February 16, 2009.

A 2-year-old boy was admitted to the hospital for febrile illness. Transthoracic echocardiogram showed dilated left (A, arrow; Online Video 1) and right (B, arrow; Online Video 2) coronary arteries. Selective coronary angiography demonstrated corresponding aneurysms of the left anterior descending (C, arrow; Online Video 3) and right (D, arrow; Online Video 4) coronary arteries. The child was diagnosed with mucocutaneous lymph node syndrome (Kawasaki disease) and underwent medical therapy with corticosteroids and intravenous immunoglobulin. However, the coronary arteries continued to dilate, and surgical repair is being considered.
