Directions for using the tool: 1) Place patient in highest clinical subgroup that describes the indication. 2) Within the selected subgroup, evaluate patient for class I, IIa, IIb, or III. ACC = American College of Cardiology; AHA = American Heart Association; CABG = coronary artery bypass graft; EF = ejection fraction; LAD = left anterior descending; LBBB = left bundle branch block; LM = left main; LV = left ventricular; MI = myocardial infarction; PCI = percutaneous coronary intervention; Prox.= proximal; PTCA = percutaneous transluminal coronary angioplasty; STEMI = ST-segment elevation myocardial infarction; 1VD = 1-vessel disease; 2VD = 2-vessel disease; 3VD = 3-vessel disease.
