Hypertension was defined as blood pressure ≥160/95 mm Hg and/or prescription of antihypertensive agents; hypercholesterolemia corresponded to plasma total cholesterol levels ≥6.5 mmol/l and/or prescription of lipid-lowering agents; obesity was defined as a body mass index ≥30 kg/m2; and smoking corresponded to active cigarette smoking at the time of diagnosis or within the preceding five years. *p < 0.05; **p < 0.01; ***p < 0.001.
