The basal angiogram (A) shows total occlusion of the left anterior descending artery (LAD) with thrombus image bulging into the left main artery (arrow). After thrombus removal (B and C), the culprit lesion (arrowheads) is observed in the mid-LAD more than 20 mm distal to the original occlusion site.
